I had the opportunity to talk with Dr. Tobinick Tuesday, January 22nd. Pertinent information obtained is as follows:
The uniqueness of the perispinal injection technique patented by him in May 2006 is that it is an extrathecal (non-spinal)approach which facilitates crossing of the blood-brain barrier.
Off-label use of Enbrel requires only agreement between doctor and patient. Dosage for Alzheimers is half that normally prescribed for rheumatoid arthritis and other pathologies.
- The procedure is presently only available to patients at Dr. Tobinick’s office in Los Angeles; however, he offers on-site training to interested practitioners there as well. Initial Six weeks of therapy recommended costs $3,600. Weekly injections are required thereafter -$180.
- I beelieve experience thus far relates only to early/ moderate (including the 81 year old) stages of Alzheimers. Similar results for other forms of dementia: vascular, bacterial/viral, Lewy bodies, trauma etc. has yet to be determined.

The quote that you received is a great deal lower than ones that I encountered. I've heard as high as $800 per injection.
Found a price quote from someone else that is for a newly opened facility in Florida administering Dr. Tobinick's treatment. The costs are below:
Here are the fees: $100.00 to reserve an appointment, then $700.00 for the physical. The neuro exam is $5,000,$600.00 for appointments plus the cost of the medicine. They are currently treating.
Regrettably...At these prices treatment is out of reach of all but a select few.

Dr. Gayatri Devi (the Dr. Tobinick in network doctor) located in NYC is rumored to be charging $9000.00 up front and in excess of $1000.00 per weekly injection. The cost of Enbrel ($188.00 per dose) may not even be included in the injection fee.
Meanwhile.... Additional research is showing that Enbrel has potential as a tool in fighting Alzheimer’s. Yue Wang (one of a group of scientists at the NIA/NIH (National Institute on Aging/National Institutes of Health) did a study to find out if Etanercept could be of value as a treatment for A/D and came to the following conclusion:

“Our findings reveal rapid, nSmase-mediated, effects of TNF on the function of two types of glutamate receptors that regulate hippocampal synaptic plasticity, and provide a potential explanation for the recently reported rapid improvement in cognitive function of AD patients treated with a TNF inhibitor (Tobnick and Gross, J Neuroinflammation 2008 Jan 9;5:2).”
